Documentation Guidelines: Reading and Seeing Accommodations

When requesting reading and seeing accommodations, documentation should demonstrate:

  • The student’s disability
  • Documentation demonstrating the impact of that disability on the student’s ability to read

For students with visual disabilities:

  • Documentation of the student’s visual disability, including a comprehensive visual examination within the past two years
  • Descriptive information from the school, such as a functional visual evaluation or Teacher Survey Form

For students with learning disabilities:

  • Documentation of the student’s learning disability, including a comprehensive cognitive and academic evaluation
  • A comprehensive measure of reading ability
  • Descriptive information from school, which could be submitted via the Teacher Survey Form

FAQ

What if my student can read, but reads very slowly?

In that case, a request for extra time may be more appropriate than a reading and seeing accommodation.
