Documentation Guidelines: Reading and Seeing Accommodations
When requesting reading and seeing accommodations, documentation should demonstrate:
- The student’s disability
- Documentation demonstrating the impact of that disability on the student’s ability to read
For students with visual disabilities:
- Documentation of the student’s visual disability, including a comprehensive visual examination within the past two years
- Descriptive information from the school, such as a functional visual evaluation or Teacher Survey Form
For students with learning disabilities:
- Documentation of the student’s learning disability, including a comprehensive cognitive and academic evaluation
- A comprehensive measure of reading ability
- Descriptive information from school, which could be submitted via the Teacher Survey Form

What if my student can read, but reads very slowly?
In that case, a request for extra time may be more appropriate than a reading and seeing accommodation.
